Monmouth, Maine: We invite an experienced Nurse Practitioner or Physician Assistant to join a Federally Qualified Health Center operating 4 clinics serving a multicultural, rural population of over 12,000 people in central Maine. Founded in the 1970s, 3 practices are located between Lewiston (second largest city in Maine) and Augusta (state capital), with their newest clinic in the western Maine town of Bridgton. Each site is about 1 hour from Portland. This respected organization offers primary care with integrated behavioral health plus care managers and a patient assistance coordinator in a nationally recognized Patient-Centered Medical Home model.Candidates certified for all ages/family medicine or adult medicine are all welcome. We have other options for new graduates or those seeking a part time schedule.Enjoy a convenient 4 day/week schedule (40 hours) practicing alongside Physicians and APPs providing compassionate outpatient primary care to people of all ages, pediatrics to geriatrics. Your patient panel will be based upon the scope of your certification. People interested in addiction medicine will receive support to certify in this area.
