We are looking for a Nurse Practitioner or Physician Assistant to join our team and provide primary care/Geriatric Care services to our patients in Assisted Living Facilities, Skilled Nursing Facilities, and/or homebound patients. The ideal candidate should have experience in geriatrics, or palliative medicine, and have a passion for providing compassionate care. The Nurse Practitioner will be responsible for performing physical examinations, ordering laboratory tests, diagnosing illnesses, prescribing medications, and providing patient education. This is an excellent opportunity for an experienced healthcare professional to make a difference in the lives of our patients.Position 1: Performing home visit in Citrus and Marion counties for some of the most in need patients in the area. These patients are unable to leave their homes due to their frail condition. Be part of a group of caring professionals who truly want to help their patients. Focus your care on a small number (9-10 home visits a day) of patients to be able to understand their individual needs. Patients will be seen in their private homes in an effort to decrease their risk for further complications associated with frequent visits to the doctors office. Must have a passion for geriatric care and want to focus on what matters most. The care of or senior population.Position 2: Geriatric care for patients residing in Assisted Living Facilities and/or skilled nursing facilities located in Marion and Citrus County, Fl. These patients are frail and elderly and truly welcome you into their homes. This population is generally very respectful to their healthcare provider as you are performing a integral role in decreasing their need to leave their communities, which will help them live a more active life and improve their overall quality of living.Responsibilities:Provide primary care to Geriatric Patients.Perform physical examinations and assessments.Diagnose and treat acute and chronic illnesses.Prescribe medications and treatments as necessary.Provide geriatric care, including long-term care planning.Educate patients on preventive health care measures and lifestyle modifications.Coordinate with other healthcare providers to ensure continuity of care.When approriate, bring the office to the patient by performing basic procedures such as IM injections, Joint Injections, and simple Dermatology procedures.
