About the Nurse Practitioner Role
As a Nurse Practitioner at Aspire Health, you will play a crucial role in impacting the lives of our patients and their families through palliative care. Collaborating with our Lead Physicians and other healthcare providers, you will develop comprehensive care plans tailored to each patient's unique needs and goals.
Responsibilities
- Provide in-home palliative care services within your designated territory
- Develop and implement individualized care plans for patients with complex medical conditions
- Educate patients and families on medication usage, illness progression, and crisis prevention
- Coordinate care with interdisciplinary team members and other healthcare providers
- Maintain accurate medical records and documentation through our EMR system
- Participate in continuing education to stay current with nursing regulations
- Prescribe medication within state regulations and Aspire Health standards
Qualifications
- Active Nurse Practitioner license in [state]
